Annual Membership Fees *

Please note that the Annual Membership Fee is due before or at the time of our Annual General Meeting (AGM). An invoice for the Annual Membership Fee will be emailed to you upon receipt of this form. We accept payment in the form of: Email Money Transfer which can be sent to treasurer@haac.ca, or Cash, or Cheque/Money Order (payable to the Health Association of African Canadians), or by major credit card (via Stripe payments).
